Forms - If you’d like to fill out our office forms at home, please print the following forms and bring them with you to your appointment.
Insurance – Ronan Eye Clinic accepts many different insurance plans. We ask that you bring your insurance cards with you at your initial appointment. While we are familiar with many of the plans, it is impossible to know the benefits extended by every plan - for this reason, it’s helpful if individuals review their benefits prior to their appointment. As a courtesy to our patients, we are happy to file your claims and provide any information and documentation that may be requested. Ronan Eye Clinic does the best to maximize your insurance benefits while providing you with the quality care unique to your needs. We do require an insurance co-payment the day of your treatment.
